Extended Access
As part of the 2022/23 contract with NHS England, GP Practices, as part of a Primary Care Network (PCN), have been asked to deliver extended access appointments (evening and weekend slots) to patients across the PCN.
These appointments will be provided by various practices within the PCN and will be open to all registered patients from the respective GP Practices within the PCN.
This means that, in order to deliver this service and provide the same level of care that you would receive from your own practice, we must share your GP records with those delivering the appointments to ensure they have access to all the clinical information about you that could be relevant to your issue. These clinicians will also record information about the consultation back into your registered GPs clinical record, ensuring that all of your primary care information is available in one place.
Partner PCN Practices that your information could be shared with are Southend East Primary Care Network
Your registered practice will remain data controller for this information, and all rights under the Data Protection Act and UK GDPR would be dealt with by them, as detailed elsewhere within this privacy notice.
Patients are able to decline this sharing, however this means that you cannot be provided with an appointment delivered by another PCN Practice in the extended timeslots, instead an appointment with your registered practice will need to be booked in normal hours.
